The American Breast Cancer Foundation (ABCF) is a national nonprofit dedicated to making breast cancer screenings and diagnostics accessible to all. Our Breast Cancer Assistance Program (BCAP) provides financial support so that cost isn’t the reason you delay or skip care.

If you are not looking for financial assistance for a screening mammogram, diagnostic mammogram, or breast ultrasound, please explore our Education & Patient Advocacy Program (EPAP). Who This Program Is For

You may qualify for BCAP if:

  • Those who have not been diagnosed with breast cancer (a prior diagnosis does not disqualify you from testing help).
  • You need a breast cancer screening or diagnostic test (such as a mammogram or breast ultrasound).
  • You are uninsured, underinsured, or struggling with out-of-pocket costs.
  • You might delay or avoid care because of financial barriers.
  • You are any age, from any background, and of any gender.
  • Having insurance does not disqualify you.

What We Cover

BCAP helps pay for breast cancer screening and diagnostic tests, including mammograms and breast ultrasounds. Approved assistance is paid directly to your healthcare provider.

  • Coverage up to $200 for screening mammograms
  • Coverage up $250 for diagnostic mammograms
  • Coverage up $250 for breast ultrasounds

Before You Apply

To complete your application, you will need to know:

  • The specific breast imaging test you need (screening mammogram, diagnostic mammogram, or breast ultrasound).
  • Generally where you want to have your test done (you can choose the provider).
  • One (1) proof of residency in your name, such as:
    • Phone, internet, or utility bill
    • Insurance card
    • Driver’s license or state ID

You must submit an application and receive a grant voucher before scheduling your testing appointment.

How the Application Process Works

1. Fill out the secure online application
Complete the application on this page before your breast cancer screening or diagnostic test.

  • A trusted family member, friend, or caregiver may apply on your behalf only if they have your explicit, written permission. Please email proof of permission to support@abcf.org.

2. Multiple tests, one application
ABCF may not require you to split up your appointments in order to receive financial assistance for more than one test. Your provider will determine whether multiple tests will be done in one visit, and if so, ABCF will provide financial support for both. If you know in advance that you will need multiple tests, please provide that information on your application. If your provider determines that you will need more than one test while at your initial screening appointment, simply let us know. In this case, we may not require a second application – our Program Manager will provide guidance on next steps.

3. Application review
Our Program Manager reviews applications within 3–4 business days and may contact you if we need more information.

  • In some cases, a doctor’s referral may be required if your chosen provider requires one, however, most providers do not for a screening mammogram. If you need diagnostic testing such as a diagnostic mammogram and/or breast ultrasound, most providers require a referral from your doctor.

4. Approval and scheduling
If your application is approved, you will be notified by ABCF.

  • Once you have been approved, you will receive your grant voucher via email. You will have 90 days to schedule and complete the appointment with the healthcare provider of your choice.
  • Once you receive your grant voucher, please schedule and complete your appointment as soon as possible. You will have 90 days to complete your test. ABCF must be billed by your provider before the grant voucher’s expiration date. If there are any issues keeping or making an appointment, please contact ABCF as soon as possible.

5. Coordinate billing with your provider
Before your appointment, contact your provider to confirm whether they will accept the ABCF grant voucher. A Grant Voucher, Grant Letter, and Grant Voucher Guide to Providers are included for you to share with them.

  • If the provider confirms the ABCF grant voucher will be accepted, please get the contact information for the provider (name, department, email, phone/fax number), in case a billing question arises later.
  • If the provider is unsure or indicates they may not accept the grant voucher, don’t worry—you may share the provider’s contact information with us so our Program Manager can reach out to provide clarification and, when possible, set up an agreement, provide clarification and, when possible, set up an agreement.

FAQ’s

Does having insurance mean I can’t get help?
No. Having insurance does not automatically disqualify you. BCAP is designed for people who are uninsured, underinsured, or struggling with out-of-pocket costs.

Do you help pay for treatment, or just tests?
BCAP focuses on financial assistance for breast cancer screenings and certain diagnostic tests (such as mammograms and breast ultrasounds). We do not cover additional treatment-related costs.

Will money be sent directly to me?
No. If you’re approved, ABCF coordinates payment directly with your healthcare provider for the approved test.

How long will it take to hear back?
Most applications are reviewed within 3–4 business days. Our Program Manager may reach out if we need more information to complete your review.

Can someone help me fill out the application?
Yes. Our Program Coordinator, or a family member, friend, or caregiver may help you complete the form.

  • If they submit on your behalf, they must have your explicit, written authorization. Please email proof of permission to support@abcf.org.

Are you able to help with any additional resources?
Our program director can provide a current and reliable list of reputable resources within the breast health network. This list of resources is given to applicants when program applications are limited or when the applicant may need additional assistance.
